When I first proposed this in “Day 15” of my “Bali Story 2000” series of diaries posted on the Bali Travel Forum (www.balitravelforum.com) it was really just a bit of a joke. 
Having now had several requests via e-mail for a copy of the form, and having recently seen a real need for it in the hands of tourists in Australia also, I’ve decided to give it free rein here.

Print off a few copies next time you’re travelling and keep them in your bum bag.   When an opportunity for comment arises, either really good or really bad, slip one out and let rip. Even if you don't find a toilet you want to test the slips might be useful for something else.

If an enterprising printer wants to commercialise the idea, making up the sheets into a pad with tear-off slips or self-carboning copies, I would like to buy the first pad.
